I attest that I am an authorized representative of Purchaser. I attest, in finalizing this Order (including each line item on this Order), that Purchaser currently possesses all required Federal and State licenses to order and receive the controlled or non-controlled substances identified in this Order. I also attest that each of the required licenses are currently valid, not expired, and not restricted or suspended. I understand that the required licenses include, if applicable, a DEA registration, State Controlled Dangerous Substances (CDS) license, and State Pharmacy license. I acknowledge that I am or the Purchaser is required to notify Taiga immediately if Purchaser has any negative action taken against a required Federal or State license. I understand the policies and procedures of the Taiga returns process for product purchased.
